What is Crepey Skin, and why is it different from ordinary wrinkles?
Crepey Skin refers to a skin surface that appears Fine, parallel, thin lines resembling crumpled paper texture, commonly seen in thinner-dermis areas such as the under-eye, cheeks, neck/chest, and upper arms. It Not expression lines, nor Not simple wrinkles— It is a “skin texture condition” caused by the triple decline of dermal collagen + elastin fibers + water content, and must be addressed at the dermal rebuilding level.
Why can’t it be treated like ordinary wrinkles?
Ordinary dynamic wrinkles (such as crow’s feet) can be improved with Botox®; however, Crepey Skin is a A dermal-layer structural problem, Neurotoxin injection is ineffective — collagen regeneration must be stimulated and dermal volume restored.
Why do Asian faces also develop it?
Although Asian skin has relatively thick collagen reserves, dermal thinning after age 40 progresses just as quickly as in Caucasian skin, and Rapid weight loss / chronic UV exposure / barrier damage significantly accelerates the onset of Crepey changes.
Why is multi-device synergy necessary?
A single device can only address one dimension. Crepey Skin involves Collagen + elastin fibers + hydration + volume multiple layers, requiring a multi-modal combination of Sculptra® + Thermage® + Rejuran®.
Why is assessing dermal thickness first essential?
VISIA + Pinch test + high-resolution ultrasound can assess dermal thickness and elasticity.Extremely thin dermal areas(such as the lower eyelid) should avoid high-energy radiofrequency and prioritize biological regeneration protocols.
Clinical highlights:Many clients are told “just get Botox for wrinkles,” only to see no improvement after three months — because what they have isn’t dynamic wrinkles at all, but Crepey Skin. Our physician’s protocol The first step is always identifying the layers involved(dynamic wrinkles / static wrinkles / Crepey / volume loss) — not simply turning on a device or starting an injection.
Five causes of Crepey Skin — it’s not just “age catching up”
Dermal crepiness is a Multifactorial overlap is a progressive process — some factors are fully reversible, while others require long-term management.
1. Intrinsic aging (collagen / elastin fiber loss)
25 After this age, dermal collagen decreases by approximately 1% per year, with elastin fibers degrading in parallel. By age 40, dermal thickness may be 20–30% less than at age 20, producing the characteristic paper-thin skin feel.
2. Photoaging (cumulative UVA + visible light)
UVA penetrates the dermis and directly damages collagen and elastin fibers, causing “solar elastosis” — Crepey Skin on the neck/chest V-zone and upper arms is almost entirely driven by photoaging.
3. Rapid weight loss / significant weight fluctuation
Rapid fat loss in a short period removes the volume support beneath the dermis, but the dermis has limited ability to retract. This is one of the main triggers for Crepey Skin in clients over 40.
4. Barrier damage / over-skincare
Chronic over-exfoliation with acids, incorrect product use, and aggressive cleansing damage the stratum corneum barrier, lowering dermal water content and producing a deflated, paper-like appearance.
5. Estrogen decline / perimenopause
Estrogen regulates collagen synthesis. Around perimenopause (ages 45–55), collagen loss accelerates significantly, and the paper-like skin appearance often emerges in concentrated fashion.
4 Skin conditions commonly mistaken for Crepey Skin — differential diagnosis key points
Misclassification equals treatment failure. The table below summarizes the 4 differential categories most commonly seen in our physician’s clinical practice.
Dynamic Wrinkles
Dynamic Wrinkles · Expression lines
| Distribution | Glabellar lines, crow’s feet, forehead lines and other mimetic muscle areas |
|---|---|
| Morphology | Faint at rest, prominent with expression |
| Dermal thickness | Normal |
| Trigger factors | Repeated muscle contraction |
| First-line treatment | Botox / Dysport Neuromodulator |
Static Wrinkles
Static Wrinkles · Deep lines
| Distribution | Deep folds such as nasolabial folds and marionette lines |
|---|---|
| Morphology | Visible at rest, deep, and presenting as individual lines |
| Dermal thickness | May be normal or thinned |
| Trigger factors | Volume loss + gravity |
| First-line treatment | Hyaluronic acid filler / Sculptra® |
Sagging Skin
Sagging · Widespread descent
| Distribution | Overall descent of the jawline, neck, and cheeks |
|---|---|
| Morphology | Descent of large tissue blocks, not fine lines |
| Dermal thickness | Normal or sufficient thickness |
| Trigger factors | SMAS + Ligament laxity + volume |
| First-line treatment | Thermage® FLX / Sofwave™ / Ultherapy® |
Crepey Skin
Crepey Skin · Crepey lines
| Distribution | Under-eye area, cheeks, neck and chest, and thin areas of the upper arms |
|---|---|
| Morphology | Fine, parallel folds resembling crumpled paper |
| Dermal thickness | Markedly thinned |
| Trigger factors | Collagen + elastin + hydration degradation |
| First-line treatment | Sculptra® + Thermage® + Rejuran® |
⚠️ Warning:Clinically, about 40% of middle-aged clients who come in requesting Botox® for wrinkles are re-diagnosed after assessment as having Crepey Skin or volume loss — neither of which responds to Botox®, and in fact may make the skin appear more deflated. Inaccurate diagnosis → failed treatment → repeated visits is the biggest pain point for middle-aged aesthetic clients.
Why does Crepey Skin in Asian skin require a dedicated Fitzpatrick III–V protocol?
Blind spots in mainstream North American clinics
Most North American Crepey Skin protocols are based on data from Fitzpatrick I–II Caucasian skin. Applying them directly to Asian Fitzpatrick III–V skin creates three core problems:
- ✗ RF energy overload— Epidermal melanin absorbs heat, raising PIH risk
- ✗ Inaccurate volume assessment— Asian facial bone support differs in structure — filler strategies cannot be copied directly
- ✗ Inaccurate reading of dermal thickness— Asian women have an overall thicker dermis, but local thin-skin areas vary significantly
Our physician’s Fitzpatrick III–V specialty calibration
| Parameters | North American standard | Asian-specific adjustment |
|---|---|---|
| Thermage® energy | 4.0–5.0 | 2.5–4.0 Staged |
| Sculptra® dilution | 5-7 mL | 8-10 mL High dilution |
| Rejuran® in-depth | 1.0-1.5mm | 0.6-1.0mm Layered |
| Interval | 4 weeks | 4-6 weeks |
| Pre-treatment | None | 4 weeks of barrier repair |
6 treatment modalities for Crepey Skin — combined by subtype
There is no single “miracle device” for Crepey Skin. We combine the devices and treatments below based on dermal thickness assessment, volume assessment, and patient budget.
Rejuran® (PDRN salmon DNA)
Targeted superficial-dermal PDRN injection directly Repair elastin fibers + rebuild the collagen network— The globally recognized gold standard for dermal regeneration in Crepey Skin.
Learn about Rejuran® →
Sculptra® (PLLA collagen biostimulator)
Poly-L-lactic acid stimulates sustained collagen regeneration over 6–24 months, suitable for Combined dermal thinning + volume loss for moderate-to-severe Crepey Skin. Our physician uses the high-dilution slow-push protocol, significantly reducing nodule risk.
Thermage® FLX
Monopolar RF deep heating causes immediate collagen contraction + long-term regeneration, with Widespread crepey laxity(jawline / neck) achieves the best results. Maintenance every 12–18 months.
Hydrating injections (hyaluronic acid + vitamin cocktail)
Superficial dermal hydration + antioxidant support to rapidly improve the “dry crepey” appearance. Used between laser/Sculptra sessions to enhance skin feel and radiance.
Sylfirm X RF Microneedling
Radiofrequency microneedling penetrates the dermis to stimulate remodeling, and for Photoaged crepey texture on the neck/chest V-zone delivers excellent results. Can be combined with Rejuran.
Fotona 4D SMOOTH Pattern
Bidirectional intraoral + extraoral laser visibly improves fine crepey lines around the eyes and mouth, and serves as a synergistic complement to Rejuran.
Our physician’s 6-step standard process for Crepey Skin
Consultation + dermal assessment
30 Minute free consultation including VISIA analysis + pinch test + volume assessment — Our three-part workup.Never turn on a device without clarifying the layers being treated— Distinguishes dynamic wrinkles / static wrinkles / Crepey / volume loss.
4 Weeks of pre-treatment prep (barrier repair + nutrient priming)
Topical retinol + ceramide barrier repair + strict mineral SPF 50+. This step significantly enhances subsequent dermal regenerative response.
Volume-layer correction (Sculptra® / hyaluronic acid, if indicated)
For clients with significant dermal thinning + volume loss, the underlying structure is restored first to provide a foundation for subsequent dermal regeneration. Performed by our physician personally, using the high-dilution slow-push technique.
Primary dermal regeneration (Rejuran® / Sylfirm X®)
One session every 4 weeks for 3-5 sessions total, with targeted injections in the superficial-to-mid dermis or RF microneedling, stimulating collagen + elastin network rebuilding.
Tightening finish (Thermage® FLX / Fotona® 4D)
After the dermal regeneration cycle is complete, Thermage® or Fotona® 4D is used for overall tightening and texture refinement.
Maintenance phase (long-term management)
Rejuran® every 4–6 months + Thermage® / Sculptra® every 12–18 months + strict sun protection. Crepey Skin is a degenerative condition that cannot be cured in a single session, but it can be kept stable long-term through maintenance.
Treatment timeline — how long until you see results?
| Severity | Primary treatment phase | Initial results | Significant improvement | Maintenance phase |
|---|---|---|---|---|
| Mild (fine crepey lines under the eyes) | 3 sessions of Rejuran® | After session 1 | After session 3 | Every 4–6 months |
| Moderate (cheeks + neck) | 3-5 Session combination | After session 2 | After session 4 | Every 6 months |
| Severe (full face + neck and chest) | Sculptra® 2–3 sessions + Thermage® 1 session + Rejuran® 3–5 sessions | Months 1–2 | Months 4-6 | Combination every 12-18 months |
| Post weight loss / perimenopause | Personalized combination over 6-12 months | Month 2 | Month 6 | Long-term combination maintenance |
Clinical note:Dermal regeneration takes time, and collagen’s visible-effect curve doesn’t begin until 6–8 weeks post-treatment at the earliest. The biggest mistake clients make is giving up after one session because they don’t see immediate change — the core of our protocol is Cumulative dermal reconstruction, It is not the illusion of immediate filling.

About the cost of Crepey Skin treatment
Pricing for Crepey Skin treatment varies significantly between clinics — but the key driver of cost is not the device or product brand itself. It comes down to who performs the assessment, who decides the treatment pacing, and how deeply clinical experience is involved. Crepey Skin involves multiple layers: dermal regeneration + volume restoration + tightening synergy. Misjudging any one layer can render the entire plan ineffective or unnatural — these cases are personally managed by our physician, and our pricing reflects that level of medical involvement. For stable clients who only need maintenance, our trained medical team can safely execute treatments under our physician’s treatment protocol, offering a more accessible option. Price differences in laser and energy-based treatments fundamentally come from the clinical experience gap between physician-performed and technician-performed procedures. For injectables such as Sculptra®, hyaluronic acid fillers, and Rejuran®, we insist that our physician performs them personally. Which plan suits you is determined by your clinical condition, not by price — specifics will be discussed at consultation based on dermal and volume assessment.
- Payment is made at the clinic, after the consultation and before treatment begins
- Nothing is paid in advance, and no deposit is required to hold an appointment
- Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China
- All first-time clients receive a 30-minute Free Consultation and assessment
Risks and contraindications — We have to be upfront
What are the main risks?
1. Sculptra® nodule risk(significantly reduced under high-dilution slow-push protocol); 2. Short-term redness/needle marks after Rejuran (resolves in 1-3 days); 3. Short-term erythema/temporary swelling after Thermage (24-72 hours); 4. Rare subcutaneous bruising (common with injectables, absorbs within 1 week); 5. PIH/post-inflammatory hyperpigmentation (moderate risk in Asian skin, significantly reduced through low-energy protocols); 6. Results below expectations (dermal regeneration is a cumulative effect requiring multiple sessions + time).
Who absolutely cannot have this treatment?
Pregnancy, breastfeeding, active autoimmune disease, oral isotretinoin use within the past 6 months, active facial infection, severe keloid tendency without assessment, pacemaker (a Thermage® contraindication), or allergy to Sculptra® components — in these cases our physician will recommend deferring treatment or choosing an alternative.
How are adverse reaction risks minimized during treatment?
5 core practices: (1) Complete dermal assessment + volume assessment is mandatory to avoid overtreatment; (2) Sculptra uses the high-dilution slow-push protocol; (3) Intervals of no less than 4 weeks; (4) Strict mineral SPF 50+ for 4 weeks before and after treatment; (5) Return for follow-up immediately if persistent erythema, induration, or any abnormality occurs.
What does the informed consent form cover?
Every client signs an Informed Consent form prior to first treatment, covering: treatment mechanism, possible adverse reactions and probabilities, alternative options, stop-treatment criteria, follow-up schedule, and Media Release (voluntary). All content is explained by our physician personally — never delegated to administrative staff.
