Horizontal Neck Lines
What are horizontal neck lines, and why do they require a different approach than facial wrinkles?
Our physician’s Real Neck Line Cases
Real before-and-after cases for this condition are currently being compiled and updated. The complete case portfolio is available for review with our physician during your consultation.
What are horizontal neck lines, and why do they require a different approach than facial wrinkles?
Horizontal neck lines (also called necklace lines or neck lines) are one or more groove-like creases running horizontally across the front of the neck. They are Dermal collagen loss + chronic epidermal dehydration + repeated platysmal pull + cumulative photoaging a complex, multifactorial presentation — it cannot simply be treated as an extension of facial wrinkles.
Why is neck skin particularly delicate?
Neck skin thickness is only a fraction of that on the face — 60–70%, Lower sebaceous gland density, a weaker skin barrier, and greater susceptibility to moisture loss mean that energy settings safe for the face may overload the neck.
Why Are Neck Lines Appearing Earlier in the Smartphone Era?
Repeatedly looking down at a phone or tablet folds the skin at the front of the neck, gradually breaking down dermal fibers. “Tech Neck” is driving a growing number of clients in their 20s to present with visible horizontal neck lines.
Why isn’t neurotoxin alone enough?
Botulinum toxin is effective when neck lines have a dynamic component, but Most neck lines are static, structural creases— A combination of hydration, tightening, and collagen regeneration is essential. Botulinum toxin alone can only address the dynamic component.
Why is it essential to differentiate static vs. dynamic lines?
Static neck lines (visible even when lying flat) reflect structural changes in the dermis and require hydration plus tightening. Dynamic neck lines (deepening when looking down or speaking) require assessment of muscle tension.Treating without skin classification are the leading causes of neck line treatment failure.
Clinical key points:Most clients present with a mixed picture of static lines, dynamic lines, and photoaging. During the consultation, our physician uses Stretch Test (head extension assessment)Assess the structural origin of each line — not just “take one look and start the device.”
5 Root Causes of Neck Lines — It’s Not Just Aging
In clinical practice, horizontal neck lines are almost never caused by a single factor — they result from multiple contributing factors accumulating over time.
1. Loss of dermal collagen and elastic fibers
25 From a certain age, collagen decreases by approximately 1% per year. Because the neck has fewer sebaceous glands and a weaker skin barrier, dermal aging in the neck runs 3–5 years ahead of the face.
2. chronic long-term epidermal dehydration
The neck loses moisture more readily than the face; chronic dehydration causes the epidermis to lose its tensile strength, and repeated folding leads to permanent creasing.
3. Repeated platysmal pulling (posture + Tech Neck)
Looking down at a phone or screen, side-sleeping habits, and repetitive facial expressions during speech cause the platysma to contract and gradually crease fixed folds into the skin.
4. UV photoaging
Most people are diligent about facial sun protection, but the neck is almost always a sun-protection blind spot. UVA chronically penetrating the dermis, accelerating collagen degradation and elastin fiber breakdown.
5. weight fluctuation / sleep position
Rapid weight fluctuation repeatedly stretches and relaxes the neck skin; long-term side-sleeping on a high pillow deepens existing lines. These behavioral causes are the most commonly overlooked part of neck aging.
4 neck conditions commonly confused with horizontal neck lines — key differential diagnosis points
Different patterns of neck aging require different treatment strategies. Misclassification leads to suboptimal results and wasted investment.

⚠️ Warning:Many clients present with a concern about “neck lines” — when in fact the underlying issue is Multiple overlapping neck aging factors. If treatment addresses only hydration of the horizontal lines while ignoring underlying laxity or vertical muscle bands, clients often feel their money was not well spent. During the consultation, our physician will provide Comprehensive Neck Aging Assessment.
Why must horizontal neck line treatment for Asian skin follow a Fitzpatrick III–V specialty protocol?
Blind spots common in mainstream North American clinics
Neck skin is naturally thinner than facial skin, and for Asian Fitzpatrick III–V skin types, the margin for error with energy-based and injectable treatments is considerably narrower. Three common pitfalls in mainstream North American neck line protocols include:
- ✗ Energy overload— Applying facial treatment parameters directly to the neck risks prolonged redness and barrier damage
- ✗ HA Injection depth too deep— Neck dermis is thin — injecting too deeply risks nodule formation or Tyndall effect
- ✗ overlooking PIH risk— Neck PIH is harder to treat than facial PIH and is frequently underestimated
Our physician’s Fitzpatrick III–V Neck Adjustments
| parameters | North American standard | Asian neck adjustments |
|---|---|---|
| energy density | Facial treatment parameters | reduced to 60–70% |
| HA Injection depth / layer | mid-dermis | superficial intradermal microdroplet |
| Per-session dosage | Standard | divided, low-dose sessions |
| Test spot | Optional | essential |
| Pre-treatment care | None | 2–4 weeks of barrier repair |
Key differentiators:Over 90% of the clients present with Fitzpatrick III–V Asian skin.
6 Treatment Options for Neck Lines — Matched by Skin Type
There is no single solution. We combine the modalities below based on your stretch test subtype classification and a comprehensive assessment of overall neck aging.
Skinvive(HA microdroplet injection)
NMPA-registered for neck and lower face use in North America Intradermal micro-droplet HA, directly improves epidermal hydration, elasticity, and radiance. Our physician uses this as Preferred foundational treatment for static horizontal neck lines, distinctly different from facial filler — the injection plane is shallower and the dosage is significantly smaller.

Thermage® FLX — Neck Mode
Monopolar radiofrequency for deep collagen tightening, suitable for Neck laxity + deep static neck lines. Reassess every 12–18 months. Best used as the structural foundation of a long-term treatment cycle.
Micro-dose Botox® to the Platysma (Nefertiti Lift)
Micro-dose neurotoxin placed along platysma points to release dynamic tension, suitable for Prominent dynamic component neck lines, as well as clients presenting with accompanying platysmal bands.
Neck collagen biostimulators (Sculptra® / diluted Radiesse®)
A diluted collagen-stimulating agent delivered via micro-droplet injection into the dermis, gradually increasing dermal thickness over 3–6 months. Suitable for deep lines + significantly thinned dermis the client’s advanced treatment plan.
Our physician’s 6-Step Standard Protocol for Horizontal Neck Lines
Consultation + comprehensive neck aging assessment
30 Free consultation: stretch test to differentiate static vs. dynamic components, manual palpation to assess skin thickness and laxity, VISIA neck imaging, and Glogau aging classification.no device activated without proper classification.
2–4 weeks of barrier pre-conditioning
Use a neck-specific barrier-repair skincare routine (with ceramides and panthenol), apply SPF 50+ daily (must contain iron oxide), and avoid high-concentration AHAs. This step significantly improves HA and skinbooster retention.
Classification as the starting point for treatment
Superficial static lines: start with Skinvive; Deep static lines/laxity: Thermage® FLX + Skinvive; Dynamic-dominant: micro-Botox® + Skinvive; Crepey texture: Rejuran® as the primary treatment.
Active Treatment Phase (2–4 sessions, once every 4–6 weeks)
Divided micro-doses, low energy settings, and precise injection depth at every step. All injectable treatments are performed by our physician personally. After each session: ice application and barrier-repair skincare; avoid vigorous neck stretching for 72 hours.
Assessment Phase (8–12 weeks after completing active treatment)
VISIA Before-and-after comparison and repeat stretch test assessment. This informs whether additional sessions are needed or whether to progress to Thermage® FLX or a collagen-stimulating biostimulator protocol.
Maintenance phase (quarterly or semi-annual, long-term)
Skinvive Once every 4–6 months; Aquagold quarterly; Thermage® FLX reassessed every 12–18 months. Complement with long-term neck SPF, postural correction, and adjusting sleep habits — low pillow, side-sleeping positions.
Treatment Timeline — How Long Before You See Results?
| Classification | Active treatment cycle | initial results visible | significant improvement | Maintenance cycle |
|---|---|---|---|---|
| superficial static neck lines | 1–2 sessions | Within 1 week | after the 2nd session | Every 4–6 months |
| Moderate static lines + mild laxity | 2–3 sessions + Thermage® FLX | After the 1st session | from month 3 onwards | every 4–6 months + annual Thermage® FLX |
| Dynamic neck lines | 2 sessions + micro-botulinum toxin | Neurotoxin: 5–7 days | 3–4 weeks | Every 4–6 months |
| Deep lines + crepey texture | 4–6 sessions (Rejuran®-based) | after the 2nd session | After the 4th session (approximately 4 months) | quarterly long-term |
Clinical Note:Collagen regeneration in the neck is 30–50% slower than in the face, and the body needs adequate time to respond. Among our clients in Nanjing, those who follow a structured protocol for six months or more report significantly higher satisfaction than those who try a single session and wait.Treating neck lines is a marathon, not a sprint.

About the Cost of Neck Line Treatment
Pricing for neck line treatments varies widely between clinics — but the real cost driver is not simply the product or device used. What matters most is who evaluates your skin, who determines the treatment pace, and how deeply clinical expertise must be involved. Neck skin is thin, has a weaker barrier, and offers less margin for error in injection depth and dosage. For complex cases — those involving both static and dynamic components, or accompanied by laxity and crepey texture — Our physician is personally and deeply involved at every step, from consultation and skin typing through to each individual injection. The pricing reflects that level of medical resource and attention. For clients who have already stabilized and simply require quarterly hydration maintenance, our trained medical teamcan safely carry out treatments under our physician’s protocol, making care more accessible. All injectable treatments are performed by the physician personally; for certain energy-based treatments, the appropriate provider is determined by the complexity of each case. The right plan for you is decided by a comprehensive neck aging assessment — not by price point. The specific approach will be discussed at your consultation based on stretch test results and skin classification.
- Payment is made at the clinic, after the consultation and before treatment begins
- Nothing is paid in advance, and no deposit is required to hold an appointment
- Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China

Risks & Contraindications — What You Need to Know
What are the main risks?
1. bruising / erythema at injection sites(common, resolves in 3–7 days); 2. Mild swelling (24–72 hours); 3. Small nodules / micro-nodules at injection sites (rare; avoidable with precise depth and dosage); 4. Rare allergic reactions; 5. Neck PIH (low-to-moderate risk in Asian skin types; rare under strict protocol); 6. Results below expectation (influenced by individual variation in neck line depth and lifestyle factors).
Who should absolutely not receive treatment?
Pregnancy, breastfeeding, known hyaluronic acid allergy, active neck dermatitis or eczema, significant sun exposure within the past 4 weeks without full recovery, severe keloid tendency not yet assessed, active autoimmune disease, or unrealistic expectations (e.g., expecting a single session to fully resolve neck lines) — in these situations, our physician will recommend deferring treatment or addressing other concerns first.
How are risks minimized during treatment?
5 Key Protocols: (1) Strict subtype classification + stretch test; (2) Start at the lower end of the dosage range; (3) Superficial intradermal microdroplet technique — avoid large single-session doses; (4) Apply SPF 50+ (with iron oxide) to the neck for 4 weeks before and after treatment; (5) Avoid vigorous neck stretching, heat exposure, and saunas for 72 hours post-treatment.
What does the informed consent form include?
Every client is required to sign an informed consent form before their first treatment. This covers: how the treatment works, potential adverse reactions and their likelihood, alternative options, criteria for discontinuing treatment, follow-up scheduling, and an optional media release. All content is explained in person by our physician — not delegated to administrative staff.
