WHAT IS IT

Hyperhidrosis treatment at our Nanjing clinic

Botulinum toxin injections temporarily block acetylcholine — the signal your nerves use to switch on a sweat gland. Injected in a shallow grid across the affected skin, they reduce sweating in that area for several months without affecting how the rest of your body regulates temperature.

In China, botulinum toxin type A is authorized for severe primary axillary hyperhidrosis— underarm sweating that persists despite prescription antiperspirant. Palms, soles and forehead are treated at physician discretion where the clinical picture supports it; those uses are not covered by the Chinese product monograph, and we say so before you book.

Every injection at Luowei Medical Aesthetics Clinic is performed by our physician personally. Sweat-gland injections are shallow and numerous — depth and spacing decide both how well it works and how long it lasts.

Try antiperspirant first. A prescription aluminum-chloride antiperspirant, used correctly at night on dry skin, is the recommended first step and resolves a meaningful share of cases. We will tell you if you have not genuinely exhausted it — injections are not the starting point.

Does it stop you sweating everywhere?

No. Only the injected area is affected. Your body has several million sweat glands; treating both underarms addresses a very small fraction of them, and thermoregulation elsewhere is unchanged. Compensatory sweating — a recognised issue after surgical sympathectomy — is not an expected outcome of localised injections.

HOW IT WORKS

How sweat-gland injections work

Sweating is triggered when a nerve ending releases acetylcholine onto an eccrine sweat gland. Botulinum toxin prevents that release, so the gland receives no instruction to produce sweat. The gland itself is not damaged — once the effect wears off, normal function returns.

Why injection depth matters

Eccrine glands sit in the deep dermis, much shallower than the muscles targeted in cosmetic injections. Placed too deep, the product reaches muscle instead of gland — which is how palm treatments end up causing temporary grip weakness. Placed too shallow, it leaks and the effect is patchy.

This is also why the number of injection points is high: each point covers a small radius of diffusion, so an underarm typically needs 15–20 evenly spaced points per side rather than a few large deposits.

Mapping before injecting

Where you sweat most is not always where you think. Before treating underarms, we can use a starch-iodine test — iodine solution and starch powder turn dark where sweat is actively produced — to outline the area that actually needs covering.

Mapping matters because dose follows area. Treating a mapped zone uses product where it works instead of spreading a fixed dose over a guessed rectangle.

What the appointment involves

A first underarm treatment takes about 30–45 minutes from arrival to leaving, most of which is assessment and numbing rather than injecting.

Stage What happens
Consultation History, severity, what you have already tried, and whether anything suggests a secondary cause. If prescription antiperspirant has not been given a fair trial, we will say so.
Mapping Starch-iodine test where useful, to outline the actively sweating zone and set the dose to the area.
Preparation Cleansing, topical anesthetic for 15–20 minutes. For palms, a stronger approach is discussed in advance.
Injection A shallow grid of small injections, typically 15–20 points per underarm. Most people describe brief stinging rather than pain.
Afterwards Cool compress. You can drive and return to work the same day.
Review A check at 2–3 weeks, when the full effect is present, to confirm coverage and top up any missed area.

Pre-care

  • Pre-procedure: Avoid applying deodorant or antiperspirant for 24 hours before your appointment.

During

  • Post-procedure: Avoid massaging or rubbing the treated area for 24 hours.
  • 48hours. Avoid saunas, steam rooms, and strenuous exercise.
  • Avoid alcohol-containing products for one week.
  • 3-5days for a noticeable reduction in sweating, with optimal results at 2 weeks.
  • Repeat maintenance every 6–8 months.

Post-care

  • Keep the treated area clean and dry.
  • Wear breathable clothing to minimize friction.
  • Avoid using irritating antiperspirants or skincare products.
  • If mild redness occurs, apply a cold compress to soothe the area.
