What is a square face? How is jawline contouring different from simple face-slimming treatment?
The term “square face” (also referred to as a wide lower face) in Asian facial aesthetics typically describes Excessive width at the jaw angle and disproportionate heaviness in the lower third of the face appearance. The underlying cause may be Masseter hypertrophy, prominent bony jaw angle, excess subcutaneous fat, and skin laxity one of four causes, or a combination of multiple causes.Jawline Contouring is a comprehensive, diagnosis-driven protocol — not something that can be resolved with a single “face-slimming injection.”
Why can’t we just come in and get facial slimming injections right away?
If the jaw appears wide due to bony jaw angle prominence, reducing the masseter to atrophy will not narrow it. If the cause is subcutaneous fat, Botox® does not target fat. And if the issue is skin laxity and sagging, injections may actually worsen hollowing in the lower cheek.Wrong diagnosis = wasted money + worsened appearance.
Why are Asian faces more prone to a square-jaw appearance?
Asian patients typically have a more laterally prominent bony mandibular angle compared to Caucasian patients, with the masseter attachment sitting further posterior and inferior. Combined with the harder-textured diet common in East Asian cuisines, average masseter thickness tends to be significantly greater than in Western populations, resulting in a squarer lower face when viewed from the front.
“Is more always better with face-slimming injections?
No. Over-injection can cause Excessive masseter atrophy, inferior displacement of the buccal fat pad, hollowing with smiling, and reduced chewing strength. Our physician’s protocol emphasizes “just enough is best” — It is better to top up at a second visit than to overdose in a single session.
Why is palpation and dynamic assessment required before treatment?
Static photographs cannot reveal the true degree of masseter hypertrophy — patients must be asked to Palpated thickness when clenching, Observing jaw dynamics during smiling, speaking, and chewing is essential for determining the precise injection points and dosage.
Clinical note:“We want face-slimming injections” is one of the most common requests at our clinic, but in our physician’s consultation room, the first step is always Distinguishing the four root causes— Muscular, bony, fat-related, and laxity-related. Choosing a treatment that doesn’t match the root cause is the most common pitfall for patients seeking jawline refinement.
5 Root Causes of a Square Face / Wide Jawline
Clinically, many patients present with what appears to be a “square face,” but the underlying anatomy can vary enormously. Misidentifying the cause is the leading reason treatments fail.
1. Masseter Hypertrophy
The most common cause. Chronic hard chewing, nighttime teeth grinding (bruxism), and habitual jaw clenching can increase masseter thickness from a normal 7–9 mm to as much as 12–18 mm. This can be effectively addressed with neuromuscular inhibition using Botox® or Dysport®.
2. Prominent Bony Mandibular Angle
The mandible has a naturally wide flaring angle — a result of genetic and developmental factors.Injectable treatments cannot address bony structural issues, Severe cases may require mandibular angle reduction surgery (not performed at our clinic; referral available).
3. Buccal fat pad prominence and subcutaneous fat accumulation
Fat accumulation in the lower face causes the cheeks to appear full and the face to look wide from the front. These patients are better suited for Dissolving injections / radiofrequency fat reduction / buccal fat pad removal, rather than botulinum toxin.
4. Skin and retaining ligament laxity (jowl formation)
40+ When jowling gradually disrupts the jawline, the lower face loses its tapered shape and becomes square. This type requires skin tightening — such as Sofwave™, Ultherapy®, HIFU, or thread lifting — not masseter injections.
5. Combined type (2–3 of the above categories overlapping)
Clinically, more than 60% of patients presenting with a square face have a composite cause — for example, masseter hypertrophy combined with a prominent buccal fat pad, or masseter hypertrophy combined with lower facial laxity. A staged, prioritized combination approach is required.
4 Lower face concerns commonly mistaken for masseter hypertrophy
Misclassification leads to ineffective treatment. The table below outlines the 4 most commonly encountered differential categories in clinical practice.
Masseter Hypertrophy
Muscular · muscular type
| Palpation | A prominent, firm bulge behind the mandibular angle when clenching |
|---|---|
| Dynamic | Visible change in the mandibular angle while speaking or chewing |
| common contributing factors | Nighttime bruxism, jaw clenching, and hard-food chewing habits |
| Frontal contour | Square lower face with posterior-inferior projection at the mandibular angle |
| first-line treatment | Botox / Dysport Masseter injection |
Bony mandibular angle (Bony Angle)
Skeletal · Skeletal / bony
| Palpation | A firm bony prominence that is palpable even when the jaw is relaxed |
|---|---|
| Dynamic | Minimal to no change in both static and dynamic facial contour |
| common contributing factors | Genetic and developmental |
| Frontal contour | Mandibular angle flare greater than 110°, presenting as a pronounced angular projection |
| first-line treatment | Injectables cannot address this · Orthognathic surgical evaluation required |
Buccal fat pad / fatty type
Fat-type · fatty type
| Palpation | Soft and movable on palpation, with no muscular tension |
|---|---|
| Dynamic | No change with jaw movement |
| common contributing factors | Weight gain or a genetically prominent buccal fat pad |
| Frontal contour | Full, rounded cheeks (“baby face”) with an indistinct jawline |
| first-line treatment | Dissolving injections / radiofrequency fat reduction / buccal fat pad removal |
Jawline laxity / Jowl type
Laxity-type · Laxity-related
| Palpation | Skin that can be pinched and lifted, indicating weakened retaining ligament support |
|---|---|
| Dynamic | A well-defined jawline when lying down that sags when sitting upright |
| common contributing factors | Age 40 · Collagen loss · Gravity |
| Frontal contour | Loss of jawline definition, drooping oral commissures, and jowl formation |
| first-line treatment | Sofwave / Ultherapy / Thread lift / filler |
⚠️ Note:In clinical practice, approximately one in four clients who request masseter injections are reclassified — after palpation and dynamic assessment — as having a bony, fat-related, or skin-laxity cause. Treating every square-faced patient with Botox® is one of the most common diagnostic blind spots in Asian aesthetics.
Why do Asian faces require Asian-specific dosing and injection points for masseter treatment?
Common blind spots in mainstream North American clinics
North American masseter injection protocols were originally developed from Western facial structure, In contrast, among Western patients: (1) masseter hypertrophy is far less common; (2) a defined, angular jawline is often the aesthetic goal rather than a V-shaped face; (3) injection points tend to be placed higher and more anteriorly, with the focus on relieving bruxism rather than contouring. Applying a North American protocol to Asian faces leads to three common errors:
- ✗ insufficient dosing— Asian masseters are typically thicker than those of European or North American patients, meaning standard Western doses often produce insufficient slimming results
- ✗ Placement too superficial or too anterior— Prone to diffusing into the risorius and zygomaticus major, resulting in a stiff, unnatural smile
- ✗ Excessive dosing results in an unnatural appearance— Overdosing at the first session leads to lower cheek hollowing six months later — aging the appearance in patients over 30
Our physician’s Asian-specialist adjustment
| Parameters | North American standard | Asian-specific adjustment |
|---|---|---|
| Injection depth | Superficial / mid-layer | Deep layer (immediately above the periosteum) |
| Injection placement | 1–2 points | 3 Equilateral triangle injection pattern (1.5 cm within the jaw angle) |
| Initial dose | Higher upfront dose for a single-session result | Conservative starting dose · Top-up after 4-week reassessment |
| restricted zone | Less commonly emphasized | The zygomaticus zone / buccal fat pad region is strictly off-limits |
| Dynamic assessment | Static photo documentation | Static assessment + dynamic assessment + palpation — the three-step evaluation |
Key differentiators:Our physician has specialized in Asian facial aesthetics at our Nanjing clinic for over 8 years, with more than 90% of the clientele being of Asian descent.
6 Jawline Contouring Approaches — Matched to Root Cause
There is no single “magic” slimming treatment. We select and combine the approaches below based on your facial type, determined through physical palpation and dynamic assessment. Energy-based options fall into two families that are not interchangeable: radiofrequency skin tightening and ultrasound lifting. Which family fits depends on the tissue layer involved, not on which device is best known.
Botox Masseter injection
NMPA A registered botulinum toxin that inhibits acetylcholine release at the neuromuscular junction, placing the masseter into disuse atrophy.
Dysport Masseter injection
Slightly smaller molecular structure with marginally greater diffusion; some patients notice onset sooner (3–5 days vs. 5–7 days for Botox®). Duration is comparable. Particularly suited for patients who have developed resistance to Botox®.
Xeomin Masseter injection
Complexing protein-free formula (“Naked Botox”), It carries the lowest risk of antibody formation with repeated long-term use, making it well suited for clients who receive multiple injections per year and prioritize long-term, consistent results.
Letybo Masseter injection
Backed by extensive clinical data from large Asian patient cohorts, offering flexible dosing and accessible pricing.
Sofwave Jawline tightening
Non-invasive ultrasound tightening, targeting Laxity-type square face— Masseter reduction alone cannot address jowling. Restoring the V-line requires jawline tightening.
Jawline filler (hyaluronic acid)
Juvéderm Volux / Volift Micro-dose filler precisely placed at jawline gaps and the chin projection point restores the V-line from a structural level, working synergistically with masseter injections for a notably improved result.
Our physician’s 6-Step Standard Jawline Contouring Protocol
Consultation + palpation + dynamic assessment
30 Free consultation: palpation of masseter thickness while clenching, dynamic assessment while talking / smiling / laughing / chewing, and VISIA imaging for baseline documentation. No classification, no injection.
Classification and treatment planning
Identifying the primary and secondary contributors among the 4 cause categories — muscular, skeletal, fat-related, and laxity-related — determines: whether to inject, which product to use, whether to combine adjunct treatments, and whether a referral is needed first.
Initial injection (conservative starting dose)
3 Equilateral triangle point placement, depth reaching just above the periosteum, with strict avoidance of the risorius band and buccal fat pad region.Performed personally by our physician — never delegated to a technician.
4 Follow-up assessment + top-up (if needed)
At 4 weeks post-injection, the masseter reaches peak response. Palpation is used to assess the degree of reduction. If results are insufficient, a precise top-up is administered at the original injection points; if results are satisfactory, the patient proceeds directly to the maintenance phase.
3 month assessment period + VISIA comparison
Multi-angle photo comparison to evaluate front-facing V-line improvement and lateral jaw angle contour. Discussion of whether to incorporate adjunctive treatments such as Sofwave™ or dermal filler.
Maintenance phase (once every 4–6 months)
Consistent maintenance is the key to lasting masseter contouring results.2–3 after treatment cycles Once the masseter reaches a stable level of reduction, treatment intervals can gradually be extended to every 6–9 months.
Treatment Timeline — When Will You See Results?
| facial typing | onset time | significant improvement | Peak result | Maintenance interval |
|---|---|---|---|---|
| Purely muscular (mild to moderate) | 5–7 early sensation at day | 3–4 weeks | 6–8 weeks | Every 4–6 months |
| Purely muscular (severe) | 5–7 days | 4–6 weeks | 2–3 Stable after treatment cycles | Every 4 months |
| Muscular + laxity | Injections: 5–7 days / Sofwave™: 60–90 days | Staged approach | 3–4 months | Injections: 4–6 months + skin tightening: 12–18 months |
| Male faces | 5–7 days | 4–6 weeks | 6–8 weeks | Every 5–6 months (faster metabolism) |
Clinical note:Many clients expect instant slimming results right after treatment — in reality, Botox takes 5–7 days to begin working, with peak results at 6–8 weeks. If you don’t notice a change at 2 weeks, please don’t rush to add more. Wait until the full 4-week mark for a palpation reassessment. Among clients at our Nanjing clinic who maintain a consistent treatment schedule, most are able to extend their intervals to 6 months or longer after 3 cycles.

Pricing for Jawline Contouring / Masseter Reduction
Pricing for masseter injections and jawline contouring varies widely between clinics — but the real cost driver is not the product itself. It comes down to who assesses you, who determines your facial type, and how much clinical judgment is required. Jawline concerns stem from four distinct causes — muscular, bony, fatty, and laxity-related — and a mismatch leads to wasted spend, worsened results, or costly revision after over-injection elsewhere. At our clinic, all injectable treatments are performed by our physician personally. That level of physician involvement is reflected in our pricing structure. For established patients who simply need periodic maintenance, Our experienced clinical team can safely assist with pre-procedure preparation under physician protocol, but all palpation assessments and injection point determinations are overseen by the physician. The difference in pricing fundamentally reflects the clinical experience gap between physician-performed and technician-performed injections (physician vs. technician). The right approach for you is determined by your classification, not by price — the specific plan will be discussed during your consultation based on palpation and dynamic assessment.
- Payment is made at the clinic, after the consultation and before treatment begins
- Nothing is paid in advance, and no deposit is required to hold an appointment
- Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China
- All new patients receive a complimentary 30-minute Free Consultation & Assessment
Risks & Contraindications — We Believe in Full Transparency
What are the main risks?
1. Localized bruising or swelling at the injection site(common, resolves within 3–7 days); 2. Temporary reduction in chewing strength(within 1–2 weeks post-injection, most noticeable when biting hard foods, resolves naturally); 3. Smile asymmetry / involvement of the zygomaticus muscle zone(Risk increases when injection points are too anterior or superficial (resolves within 1–3 months as the product metabolizes); 4. Excessive muscle atrophy leading to lower cheek hollowing(When dosage is excessive or treatment cycles are too frequent (requires a rest period to recover); 5. Rare cases of allergic reaction / headache / flu-like symptoms(resolves within a short period); 6. In rare cases, some clients may develop antibodies, which can reduce the effectiveness of subsequent injections.
Who is absolutely not a candidate?
Pregnancy, breastfeeding, allergy to botulinum toxin or any formulation excipient, neuromuscular disorders such as myasthenia gravis, Lambert-Eaton syndrome, or ALS, active infection at the injection site, a purely bony mandibular angle (where injections are ineffective), or unrealistic expectations (believing one treatment permanently slims the face) — in any of these situations, our physician will recommend deferring treatment or referring for a surgical evaluation.
How can adverse reaction risk be minimized during treatment?
5 Core Principles: (1) Palpation and dynamic assessment are mandatory before every session; (2) Start conservatively, reassess at 4 weeks before adding more; (3) Strict 3-point deep periosteal placement, avoiding the zygomaticus (smile muscle) zone; (4) For 4 hours post-injection, avoid pressing the area, laughing hard, vigorous exercise, or inverted yoga poses; (5) If you notice asymmetry in your smile or any swallowing discomfort, book a follow-up immediately.
What does the informed consent form include?
Every patient signs an informed consent form before their first injection. It covers: mechanism of action, possible adverse reactions and their likelihood, alternative options, criteria for discontinuing treatment, follow-up scheduling, and an optional Media Release. Our physician walks through every point personally — this is never delegated to administrative staff.
